Topme XL primarily works by blocking beta-adrenoreceptors in the heart, and is used to manage hypertension, chest pain (angina), abnormal heart rhythms, as well as to treat and prevent myocardial infarction. Topme XL affects the autonomic nervous system, which leads to an increased heart rate by blocking its receptors, thereby reducing excessively rapid heart rhythms. Additionally, it decreases myocardial irritability and cardiac output, resulting in other effects.
 
Topme XL should be taken with food or shortly after a meal. Do not chew, crush, or break the tablet; instead, swallow it with a glass of water. The dosage varies based on the condition being treated and can range from 100 to 450 mg daily. Your specific dosage should be determined by your doctor. Older patients are advised to begin with a daily dose of 50 mg. Kidney failure does not necessitate dosage adjustments, but liver failure may require the use of alternative beta-adrenoblockers that are not metabolized by the liver. If you miss a dose, take it as soon as you remember. If your next dose is due in less than 4 hours, skip the missed dose and continue with your regular schedule. Do not take an extra dose to make up for the one you missed.
 
While many individuals may develop tolerance to Topme XL, some common side effects include abdominal cramps, fatigue, insomnia, nausea, depression, memory impairment, fever, impotence, dizziness, slow heart rate, low blood pressure, cold extremities, sore throat, diarrhea, constipation, and difficulty breathing or wheezing. 
 
If you experience symptoms such as irregular heartbeats, shortness of breath, dizziness, weakness or fainting, or bluish-colored fingernails, seek medical attention immediately.
